so i have this sentence, which needs a reference and appendix in parenthesis after it:
Participants were asked to answer 14 questions from the Student Alcohol Questionnaire.
i'm thinking it would look something like this:
Participants were asked to answer 14 questions from the Student Alcohol Questionnaire (reference goes here, Appendix A).
now i need your help...first, if you could tell me if the above sentence would be correct. second, how should i reference this: http://www.indiana.edu/~engs/quest/saq.html (the actual test on the site, NOT the website). only thing i can think of would be: (Engs, 2002) or maybe (Engs, Ruth C., 2002).
i've looked in the APA manual and didn't find anything with this similar situation. any help would be appreciated

In this case, (Engs, 2002) is indicated.
and just for kicks, the biblio would look something like:
Engs, Ruth C. (2002). The Student Alcohol Questionaire. Retrieved on 10th November 2005 from the World Wide Web: http://www.indiana.edu/~engs/quest/saq.html
