Basically, what happens? Do they just ask you how things went, etc. or do you have to do other stuff, like fill out paperwork?

And this paper I have to write, what needs to go in it?

Easy stuff. Just a quick (5-10 mins) meeting w/ your coordinator about how you felt about the company, do you think NCSU should keep working w/ that company, how were you treated, did you feel you learned anything, etc. Basically it's a real-life perspective on how well that company is doing and to make sure NCSU needs to keep working with them.

Paper is nothing really...just write a brief summary of your job task(s), what you enjoyed and what you disliked, and descibe if you'd recommend that job to another student and what that student should be interested in doing before committing to your specific position.

You should already have filled out a form on how you think you did that semester. You should have also given a form to your mentor to fill out. Give both of these to your coordinator along with the paper.

Is there a specific format to the paper? As in name, major, when the co-op was, with who, etc, and should it be a cover sheet or just at the top of page one.

you can see some example papers from previous co-ops in your major. they are available in the co-op office library
