People make mistakes. People in general suck at driving.
It's not like that's the last time you'll ever almost have been in an accident.

Don't give your name and who you work for. Just report yourself as an anonymous driver.

That doesn't mean we should let them drive poorly. There is a reason you can report bad drivers.

No it wouldn't be unprofessional. If you had been some mother with kids in her car who had almost been hit, you can guarantee the company would get a call if she could see the name or number on the truck.
