The closest thing to World War III will probably occur in Eastern Asia. Other regions don't have nearly the same combination of large militaries, sophisticated weapons systems, powerful economies, firey nationalism, and multiple flashpoints needed to create the massive scale of violence required to rival the previous World Wars.
It may be Korea, but that could easily be kept within the penninsula; I think the Chinese will to back the North Koreans directly is beginning to fade (or they may just invade themselves if the North Korean regime collapses). I would bet on an escalation of the Taiwanese crisis to lead to the massive bloodletting and hundreds of thousands of military casualties and millions of civilian deaths and general global economic disruptions required to qualify as a full World War.
The Middle East is full of the factors to motivate a war, but I don't think that the current governments have the firepower and organization required for sweeping advances. I would imagine more like something that we have in Iraq right now or the long, drawn out skirmishes that made up the original Gulf War (Iran-Iraq War). Unless the Israelis start dropping nukes. Then God help us all.
Oh, need to mention India and Pakistan as well. If those two decide to finally do a full nuclear throwdown, then things would get very messy very quickly. However, they seem to be doing a better job of managing tensions these days, so one can hope...
