So I think this came up during the USC/Stanford game. A guy was screaming for a horse collar penalty, then on the replay it looked like the runner may have stepped out BEFORE the penalty.

My questions is, if a challenge reverses a play to BEFORE a personal foul, is the flag picked up?

College and NFL answers accepted.

maybe not that kind of penalty, since it's to protect a player from injury

A horse collar foul can be called even after the runner steps out of bounds.

I believe the answer to your bigger question is yes. For instance a block in the back after an interception would be waved off if the interception was deemed incomplete on replay.
