For each 18 ounce (or smaller) unit of ACT Total Care Unit purchased, Settlement Class Members are entitled to a payment of $1.50 from the Settlement Fund. For each 33 ounce unit (or larger) of ACT Total Care Unit purchased, Settlement Class Members are entitled to a payment of $2.00 from the Settlement Fund. Settlement Class Members may not seek reimbursement or a claim for more than ten (10) units purchased. To receive the cash payment, the Settlement Class Member must timely submit a simplified Claim Form.

Settlement Class Member Award Increases: If any amount remains in the Net Settlement Fund after the payment of Eligible Claims, each Authorized Claimant’s Award shall be increased on a pro rata basis, with a maximum increase of up to, but not more than, $4 for each 18 ounce (or smaller) unit of ACT Total Care purchased, and $6 for each 33 ounce (or larger) unit of ACT Total Care purchased, with such aggregate awards not to exceed the Net Settlement Fund.
