Margins on that incision do not look like a resuscitative thoracotomy, but it is in the correct location, so who knows. Doesn't look like any of the ones I've seen. Looks like at least 4 GSWs, but otherwise nothing that looks too bad. Likely punctured lung (decompression needles is exactly correct) and severed renal artery based on the entry wound below the thoracotomy incision. IV would be used to push epinephrine if there was any attempt to revive (surprised it wasn't a femoral access point, but can't see that either so they may have done that as well).

Glad he's dead.

Best guess: emergency thoracotomy but it looks like it was done quickly because of the incision margins

Went back and looked at the picture. Looks like the incision was made straight across (very straight line near the middle of the body), but it didn't match up with the direction of the ribs. The rest of the wound was likely caused by the rib spreaders, as there probably wasn't much time to do anything.

Is his upper body purple because that's where he was run over? Or the blood was pushed there by the tire elsewhere?

Purple is caused by blood pooling. Was likely lying face down or slumped over at some point. The wounds from being run over wouldn't be easily visible - especially with all the other damage. Getting run over usually causes internal injuries.

peripheral iv would not be used to for vasopressors, i/e epinephrine, IO or femoral line would be done for that, as for everything else I concur
