Brush: at least once. mostly twice.
Floss: once a week OR whenever i eat beef roast or popcorn
Checkup: Used to go twice a year. Haven't been in 2 years.
Cavities: 4. I've got one now though, I can feel it.

Brush: once a day normally, but if I'm going out or had something sugary late I will brush again.
Floss: once every two weeks to a month
Checkup/ cleaning: Once a year
Cavities: 0

I brush for a good 3 minutes normally. I use mouthwash for in between cleans if need be.
I chew ice like there is no tomorrow.

Brush: Twice daily (sometimes more)
Floss: Weekly (should do more)
Checkup/Cleaning: Once a year
Cavities: One last year (24yo) (not exposed, inside the tooth)

The biggest difference in my personal hygiene has come from a new electric tooth brush that encourages me to brush for closer to 3 mins. And this has made a huge difference, recommend to everyone $2 and I promise you won't regret it.

I'm so jealous of the people who have never had cavities. I had a bunch as a kid in my baby teeth, and a few as an adult. But I brush at least twice a day, very rarely drink soda, use Listerine and fluoridated mouthwash, floss at least once a week, and up until the last year or so was religious about going every 6 months for cleanings.

Yeah, I believe brushing your tounge is one of the keys here. I try to go back as far as I can too. Sometimes I can brush my teeth in the morning and I can still feel "fresh" 12 hours later before dinner.

Drink a ton of soda, tea, coffee, etc.

Brush: 2x a week...maybe. Sometimes once or none.
Floss: A couple times to toughen my gums up in the days before my wife makes me see the dentist.
Checkup: None for 6 years, then once since being married. Probably be once every two years or so, if I have a say in it
Cavities: Never had one, and no dental work needed of any kind except for braces.

My last checkup a few weeks ago after 6 years of this routine - the dentist gushed over my oral upkeep

Dentist always says my teeth look great. My wife, who would know better than anyone, never has a problem with my breath. She brushes many times a day, flosses, checkups and cleanings every 6 months, and is riddled with cavities

I am invincible. If breath, cavities, etc. ever become a problem, I'll change. But until then, why?
