oooh, hemicraniectomy (just skull missing) they did this to give his swelling brain some room to um, swell.


AAAAAAAANNNNNNNNd it looks like he has some paradoxical herniation from a csf leak.

if they stop the leak, lay the head of his bed down and tilt the bed so that he is "on his head"

that hole will fill out again.

may or may not improve his neuro exam tho.....it depends on how much and how long his brain stem was compressed


edit* just noticed he's not on a ventilator and has a passy muir valve( for speaking) so he isnt THAT comatose....

these are the types of patients i have.



three nights ago the nurse practitioner and i were going "oh shit this is bad" to this morning we did the "wave" we were all so happy that she is going to recover. even the attending -- who is about the most uptight person working there. she is even going to have the use of her left side. Her surgeon said going into the surgery that she would probably lose function on that side.



how do you tell someone in their early 20's that if we dont remove the AVM it might rupture again and kill you this time, but if we do you will probably be paralyzed on one side. :-/

luckily for her her first child was delivered via C-Section... the avm(that she didnt know about) would have killed her during a regular delivery.


I ran around like a one legged man in an ass kicking contest for 3 days, but man it was worth it -- hooray for success stories....neuro doesnt have too many of those

in the short time ive been looking up info.......a lobectomy wasnt mentioned..

its possible some tissue was removed.

at some point in the future, if his family wants it, they can do a cranioplasty and replace his skull( assuming they saved some of it...

i am surprised he isnt wearing a helmet.


we cannot turn a hemicrani patient on their side if they dont have a helmet on.
