My thoughts:
I think the difference in treatment for thin people can partially be attributed to attractiveness. For better or worse, people tend to be warmer and friendlier to attractive people, who are generally thin.
I also think shaming fat people is counterproductive, but it's irresponsible to avoid bringing up someone's weight problem during a routine doctor's appointment. Ultimately, people have to responsible for their own health, and if they can't be bothered to follow their doctor's advice then whatever happens is on them.
