There was a policy in place to avoid this kind of thing, but it didn't prevent this incident.

Some stupid kid with special circumstances didn't tell anybody, and didn't complain when he could have, now he's dead.

All the kid had to do was speak up and he'd have gotten some water.

No coach would deny a kid some water or a break if he thought there was a legit risk. Coaches are going to push the kids and make them not be pussies, but if there is something serious, they'll do what they need to do.

The kid should have spoken up.

You obviously never played football then. Most coaches would tell you to shut up and keep running if you asked for water. A lot of them still believe that drinking water causes cramping and intentionally prevent their players from drinking water during practice.
