Why do people say "gay" when they mean "effeminate" or "sissy"?

"Gay" used to mean "happy", but now it means "homosexual", but only some homosexuals are effeminate.
When I hear it used pejoratively, "gay" is usually meant as "effeminate" or "sissy", and not "homosexual".

This male cheerleader may or may not be homosexual, so why use "gay" to describe his sissy cheerleading?
