Anyone seen software that does the following well:

Situation - Need to easily track and record my influential work. I work mostly using web based tools, so I need something that would ideally be a browser plugin, which allowed me to click a bookmark/icon and it would open up a text box, where I could write a note, and would save the url of the currently open window/tab .... Tying the note to the URL. It would then save this combo of info in a organized manner where it could be searched/sorted later.

I use multiple tools and engage in multiple areas of work where I use my knowledge to influence the success of products/customers/developers/etc.

I just want a quick and easy way to track the work I do, as there are not really tons of great metrics out there to accurately measure this sort of free flowing influence.

Any ideas ??
