Tribeca Tavern does this. I got my burger rare and it was amazing.

I like my burgers well and with char. That being said:

1) Bada - believe it or not they are fantastic
2) melvins
3) char-grill
4) five guys

Places that grind meat are Spirits, PR's, and Backyard Bistro (actually gets the meat ground daily from Angus Barn)
