Everyone in my family (on both sides) has only a first, middle, and last name.

However, my cousin just had child #2 and decided to give her baby girl two middle names because she wanted to incorporate family ino the names, and I guess didn't wanna leave out one family.

Soooo the first name is Avery (was a fam name) Melissa (her husband's late sister's name) Bell (my great-grandma's maiden name) __________ (last name).

One of my grandfathers had two middle names, but one was just a letter. He was named after someone else, which I'm not even going into because it's weird, and they used the middle initial instead of the whole name. I don't know why.

The letter is kinda odd, but not uncommon. An old friend of mine is named J (and that's his first name!) and the S in Harry S. Truman didn't stand for anything. His middle name was simply S.
