done...a few notes:

on the "Which of the following events, if any, would you be most willing to participate in (check all that apply)?" question, it says "dace-a-thon" instead of "dance-a-thon"

on the "Please indicate the highest level of education completed." question, not every masters is an MS

on the "Were you born in Raleigh, NC?", why can I select both yes and no? i think you wanted radio buttons

Or something to that effect...as Westicles said, it could be worded better
