i've heard it reported a few times that this is Skanska's first fatality in the state, but I don't think that's not true. I think they had someone die when swinging an extended boom truck across a highway sometime ago.

Skanska has a very good safety program though. In general state jobs like this one are harder for any contractor because they often have to use low-bid minority contractors despite safety records, but this concrete contractor seems to have a good reputation (and I've seen concrete blow out a local spot on a pan, but never take down something). IMO this looks like a design (or shoring?) failure.

^^^ That's what I was thinking as well. When I worked for a local cast-in-place concrete subcontractor they had a pretty thorough shoring and reshoring design procedure for this exact reason.
